Patients may drink alcohol whilst taking long-term low weekly doses of methotrexate (25mg or less) for skin conditions, rheumatoid arthritis and other inflammatory conditions, but they should be advised that both alcohol and methotrexate can potentially damage the liver, so they should not drink more alcohol than recommended by national guidelines (currently 14 units a week).

The reason for the prohibition on alcohol is that methotrexate can temporarily impact on your liver function and alcohol can make this worse. Your levels should be well down within a few weeks, so it shouldn't be too long to wait.

Our general rule of thumb at the Hopkins Arthritis Center is to allow two drinks per week. That assumes that the liver function tests are being monitored regularly. There aren’t any data that would address your question about taking the alcohol on the same day as the methotrexate but it would seem prudent not to do so.

So technically, it’s probably not a good idea to drink any alcohol while taking methotrexate. That being said, drinking one standard glass of alcohol once a week probably is safe. This is a bit of a touchy subject, and you should discuss this more with your doctor.

My rheumatologist said it's okay to enjoy (a glass at a time) wine 2-3x weekly. I take plaquenil and methotrexate. I take mtx on Wednesday and don't drink until Friday. I was told that if my liver enzymes increase, I lose the wine.😲
